Requirements for Athletic Participation

For each season, Wahkiakum High School athletes must submit all the paperwork listed below to the team's head coach and meet all the eligibility requirements before the first practice in order to participate in a sport during that season.

For eligibility in middle school sports, please refer to the Middle School Athletics Page.

ASB Card
$60 pay-to-participate fee for high school athletes
Confirmation of Receipt of Athletic Code (Athlete's and parent's signature required)
Download the Wahkiakum High School Activity and Athletic Code Handbook
Confirm Receipt of the Athletic Code Handbook with this Signature Form (Athlete's and parent's signature required)
Physical Exam Form, Medical Insurance, and Emergency Contact Info (Physician's, athlete's, and parent's signature required)
Wahkiakum Concussion Waiver (Athlete's and parent's signature required)
